1. How do I get a copy of a fire report?
If you are requesting a fire report for yourself, you may call 630-466-4513 to be sure your report is ready to be released. The completed report can be picked up from the Fire District Office, 25 S. Municipal Drive Sugar Grove, IL, with valid identification. If you are requesting a fire report for anything other than your property, a Freedom of Information request form is required for release.  Please fill out the following form and email the completed form to foia@sugargrovefire.com.  FOIA Request Form
2. How do I get a copy of an ambulance report?
If you are requesting an ambulance report for yourself, an ambulance report is considered a medical record and can be released to the patient, with proper identification. If you are requesting an ambulance report on behalf of someone other than yourself, a proper release form signed by the patient is required before the report is released. Please contact the Fire District Office for assistance, 630-466-4513.

5. What is the Fire Department’s Insurance Service Office (ISO) rating?

Currently, the Sugar Grove Fire Protection District maintains a fire protection rating of “Class 3/3Y” though the Insurance Service Office.
